Welcome to Center Street Community Health Center, a community-based and patient-directed healthcare facility that provides accessible, high-quality medical, dental, optical, chiropractic, pharmacy, and behavioral health services for all ages. We look forward to serving you with your healthcare needs. We are a Federally Qualified Health Center (FQHC) that will accept uninsured individuals as well as Medicaid, Medicare and Commercial Insurance. If you do not have health insurance, we offer a sliding fee scale which is based on family size and income. We also have staff members that can assist you with enrolling in health insurance or Medicaid if you qualify. We are a Federal Tort Claims Act (FTCA) facility.
